Work Environment

This role is entirely remote, leveraging telehealth technologies to connect with patients across various settings, including their own homes. Key Responsibilities

Your responsibilities will include a dynamic range of daily tasks, aimed at delivering high-quality mental health care:

• Conduct thorough reviews of patient health histories and prepare for comprehensive evaluations to tailor individual treatment plans.
• Develop, implement, and continually assess mental health care plans while adhering to industry standards and company protocols.
• Engage with patients and their families through interviews to identify educational needs, formulate treatment goals, and create action-oriented plans of care.
• Facilitate educational workshops, such as our healthy minds workshops, to enhance patient knowledge and engagement, fostering a supportive community even in a virtual setting.
• Evaluate the effectiveness of educational interventions and adjust care plans as necessary to ensure optimal patient outcomes.
• Collaborate closely with other healthcare professionals, including physicians and interdisciplinary team members, to coordinate comprehensive care.
• Maintain stringent adherence to patient confidentiality in accordance with HIPAA regulations and ensure all documentation is completed and stored appropriately.
• Actively make referrals to other healthcare specialists when necessary based on evolving patient needs.

Patient Population / Client Focus

You will primarily support adult patients participating in mental health programs tailored for those with cardiac conditions, with a strong focus on addressing their unique mental health needs. Our goal is to provide a holistic approach that encompasses both physical and emotional well-being.

Qualifications

To be considered for this position, candidates must hold one of the following licenses: LMHC, LCSW, LMFT, or a Psychologist license. Additionally, candidates are expected to demonstrate strong interpersonal skills, an empathetic approach to patient care, and a commitment to ongoing professional development in the field of mental health.

Preferred Experience

Experience working in a telehealth environment or with patients facing chronic health issues is advantageous. Familiarity with managing multiple patients in a virtual capacity and conducting educational workshops will be beneficial to your application.

Compensation and Benefits

This position offers a competitive salary in the range of USD 90,000 to 104,000 annually, depending on the number of patients seen each week. Additionally, we provide a comprehensive benefits package that includes:
