Work Environment\n

This role is set in a remote hospice care environment, allowing you the flexibility to manage your schedule while ensuring children and their families receive the support they need during critical moments. While the work is conducted remotely, you will integrate with teams regionally and across various programs aimed at delivering high-quality, compassionate care.

Key Responsibilities\n

• Conduct comprehensive evaluations and assessments tailored to the needs of children in the hospice program.

• Participate actively in the development and implementation of Individualized Family Service Plans (IFSPs) that are customized for each child's specific situation.

• Monitor and track developmental outcomes as part of a supportive team, collaborating with healthcare professionals to ensure holistic care for the children.

• Provide essential support and resources to families, empowering them in their caregiving roles and addressing their needs.

• Utilize a blend of evidence-based practices and Natural Learning Environment strategies to foster skill development in children during their crucial early years.
