This role predominantly operates within a remote setting, allowing for flexibility and the opportunity to connect with families across various geographic locations. Our Hospice Center provides supportive infrastructure for assessments and evaluations, ensuring that every social worker has the resources needed to thrive. You will work collaboratively with other professionals, utilizing digital tools for virtual meetings and project management to streamline team communication and service delivery.

Key Responsibilities\n

• Conduct comprehensive evaluations and assessments to determine eligibility for early intervention services.

• Play an active role in the development and implementation of Individualized Family Service Plans (IFSP), collaborating closely with families and the multidisciplinary team.

• Monitor and report on outcomes for enrolled families, ensuring adherence to both federal and state regulations.
\n
• Foster rapport with children and families to create an effective and supportive learning environment.
\n
• Utilize evidence-based practices to develop joint plans with families that reflect their priorities and concerns.

• Implement the IFSP, recommending services based on child development status and family goals.
\n
• Provide families with essential information, skills, and emotional support to enhance their child’s development.

• Embrace the coaching model and Natural Learning Environment Practices during each session.
\n
• Ensure compliance with health and safety standards for both children and staff members.

Patient Population / Client Focus\n

This role is centered around serving children from birth to three years old who exhibit developmental delays or disabilities. Our Early Intervention services aim to assist children in achieving developmental milestones such as physical, cognitive, social/emotional, and communication skills. You will be instrumental in facilitating growth and learning, providing families with the tools and strategies they need to nurture their child's development effectively.

Qualifications\n

• A Master's Degree or Doctorate in Social Work from an accredited institution.

• A valid LCSW certification licensed by the Department of Public Health in Colorado.

Compensation and Benefits\n

The annual salary for this position ranges from USD 75,000 to 80,000, commensurate with experience and qualifications.
