Key Responsibilities\n

• Engage in care system processes to implement strategies aimed at reducing readmissions and ensuring seamless transitions for patients across different levels of care.

• Conduct thorough transition conferences with patients and families, focusing on personalized care planning and resource linkage.

• Employ digital tools and technology to identify, monitor, and assist patients at risk for readmission, ensuring timely interventions.

• Guarantee comprehensive clinical handoffs for at-risk patients, which may include referral to additional services.

• Facilitate collaboration and communication with an interdisciplinary team, anticipating patients’ needs and progressing care plans accordingly.

• Provide information and support to patients regarding transition plans, financial assistance, and social services as needed.
\n
• Maintain compliance with reporting requirements related to patient rights and vulnerability, including mandatory reporting of suspected abuse.

• Document and communicate patient progress towards established goals and the evolution of their individualized social work plans within our electronic medical records system.

• Perform other duties as assigned that further the holistic approach to patient support and care.

Compensation and Benefits\n
