Key Responsibilities\n

• Manage a diverse caseload of clients, establishing rapport and cultivating positive working relationships while upholding professional ethics and boundaries.

• Conduct regular sessions with clients to assess their progress, address challenges, and coordinate upcoming appointments and discharge plans when appropriate.

• Develop, implement, and continuously monitor individualized person-centered treatment plans in collaboration with clients, ensuring a focus on their unique needs and circumstances.

• Maintain accurate and timely documentation, including comprehensive progress notes for each client, to reflect care delivered and meet regulatory requirements.
