Key Responsibilities

• Conduct comprehensive evaluations of patients' physical functioning, assessing mobility, strength, and overall physical health to establish personalized treatment plans that align with their rehabilitation goals.
• Document evaluations, progress notes, and treatment sessions meticulously in accordance with industry standards and facility policies to ensure continuity of care and compliance with regulations.
• Implement therapeutic interventions that may include manual therapy, therapeutic exercises, and modalities aimed at improving mobility, reducing pain, and enhancing strength.
• Communicate effectively with patients and their families, providing education on treatment goals, methods, and progress to foster collaboration and support throughout the rehabilitation process.
• Regularly evaluate patient progress and adapt treatment plans as necessary to ensure optimal outcomes, documenting changes clearly and communicating updates to relevant stakeholders.

Patient Population / Client Focus

You will work with a diverse patient population, including children, adults, and seniors, each requiring specialized approaches to physical therapy based on their individual needs and conditions. By leveraging your clinical expertise, you will provide effective, evidence-based interventions tailored to each patient's unique circumstances and rehabilitation goals.

Qualifications

• A Bachelor's degree in Physical Therapy or a Doctorate in Physical Therapy (DPT) from an accredited program.
• Current and active Physical Therapist license in the state of California.
• Demonstrated clinical expertise, including mastery of various manual therapy techniques and intervention methodologies.
• A clean professional record with no history of sanctions or board proceedings.
• Exceptional interpersonal and communication skills to foster strong relationships with patients and their families.

Preferred Experience

• Previous experience working in a home care setting or a similar clinical environment.

Compensation and Benefits
