Work Environment

This position offers a unique opportunity to work primarily in a home care setting, where you will provide services to patients directly in their homes. Key Responsibilities

• Conduct thorough evaluations to determine patient needs and develop individualized treatment plans that encompass manual therapy, exercises, and the use of assistive devices.
• Administer therapeutic interventions and consistently monitor patient progress while modifying treatment plans as necessary based on patient feedback and clinical outcomes.
• Educate patients, their families, and caregivers on home exercise programs and strategies to facilitate recovery, ensuring clear understanding and engagement in the therapeutic process.
• Supervise and guide paraprofessional staff in implementing physical therapy treatments, fostering a collaborative environment for optimal patient care.
• Participate in our mentorship program, providing guidance to newer therapists and contributing to their professional development as well as your own.
• Engage in quality assurance initiatives within the department, continually assessing and enhancing service delivery standards.
• Stay abreast of the latest research and techniques in physical therapy, integrating emerging insights into practice to improve patient outcomes.

Patient Population / Client Focus

As a Physical Therapist at Twin Mountain Physical Therapy, you will work with a diverse population of patients at varying stages of recovery, from post-operative rehabilitation to chronic pain management. Our home care focus allows you to build meaningful relationships with patients, providing personalized attention that addresses their specific challenges, goals, and progress in a supportive manner.

Qualifications

• Licensed Physical Therapist in good standing, with current licensure in California.
• Possession of a Bachelor's degree in Physical Therapy (for those graduating before 2002) or a Master of Science/Doctorate in Physical Therapy (for those graduating after 2002).
• A minimum of two years of experience in a relevant role is preferred.

Preferred Experience

• Experience in home health settings, demonstrating a strong ability to work independently.
• Familiarity with telehealth practices and providing care through virtual platforms.
• Proven record of actively engaging patients and families in their care plans to support successful health outcomes.

Compensation and Benefits

At Twin Mountain Physical Therapy, we offer a competitive salary range of $110,000 – $120,000 per year, commensurate with experience. In addition to salary, our benefits package includes:
