Work Environment

As a Physical Therapist at Guided Grace Therapy Center, you will work onsite, primarily in a home care environment. This setting allows you to build meaningful relationships with your patients while providing therapy tailored to their unique needs. You will work within a team of healthcare professionals, including physicians, nurses, occupational therapists, and speech-language pathologists, to ensure a comprehensive approach to patient care. Your work schedule will generally be Monday to Friday, with some rotating shifts on weekends to accommodate patient needs.

Key Responsibilities

In this role, your responsibilities will include:

• Conducting thorough evaluations of patients to determine their physical capabilities and limitations, setting measurable goals, and designing individualized treatment plans based on physician referrals.
• Delivering hands-on therapy treatments, utilizing techniques and modalities that are evidence-based and appropriate for each patient's condition.
• Maintaining accurate and timely documentation of patient care activities including evaluations, treatment notes, and discharge summaries, ensuring compliance with organizational and regulatory standards.
• Monitoring patient progress and adapting treatment plans when necessary, demonstrating flexibility and responsiveness to patients' needs throughout their rehabilitation journey.
• Providing education and support to patients and their families about conditions and rehabilitation processes, fostering understanding and engagement in care plans.
• Supervising and mentoring Physical Therapy Assistants (PTAs) and support personnel to promote a cohesive team approach to patient care.
• Collaborating with interdisciplinary team members to integrate therapy services into comprehensive care plans, ensuring continuity of care and optimal patient outcomes.
• Contributing to departmental strategies aimed at improving productivity, quality of care, and patient satisfaction through feedback and innovative solutions.

Patient Population / Client Focus

Our focus is on a diverse patient population including individuals recovering from orthopedic surgeries, sports injuries, neurological conditions, and chronic pain syndromes. You will have the opportunity to work closely with patients of varying ages and backgrounds, each with unique needs. Your role will be key in helping them regain independence and enhance their quality of life through targeted interventions and compassionate care.

Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited institution.
• A current Physical Therapist license issued by the state of Virginia.
• Strong verbal and written communication skills, enabling you to articulate treatment plans clearly to both patients and team members.
• Proficiency in word processing and electronic documentation, ensuring efficient record-keeping and communication.

Preferred Experience

We are seeking candidates with at least two years of experience in a clinical Physical Therapy setting, preferably in home care or similar environments. Experience with a variety of patient populations and treatment techniques will be advantageous as you customize therapies to meet specific recovery goals.

Compensation and Benefits
