Work Environment

This position is designed to be fully remote, allowing for flexibility in your work environment while still providing comprehensive support to our community clients. Our organization utilizes a range of digital tools and systems to facilitate communication and service delivery, ensuring that you can effectively collaborate with fellow team members and provide timely support to clients. Standard work hours are Monday to Friday, from 9:00 AM to 5:00 PM, enabling a balanced work-life integration.

Key Responsibilities

• Serve as a liaison between families and outpatient providers, ensuring that clients receive coordinated care tailored to their individual needs.
• Engage with insurance companies to facilitate pre-certification processes and manage utilization effectively, ensuring proper coverage for our clients.
• Participate actively in discharge planning and aftercare strategies, assessing ongoing needs and connecting clients with appropriate resources.
• Provide therapeutic meal support that emphasizes cultivating a healthy relationship with food, utilizing evidence-based practices to engage clients positively.
• Deliver empathetic, safe, and affirming care that acknowledges and values each client’s intersecting identities, fostering trust and rapport.

Patient Population / Client Focus

Qualifications

• Master's degree in Social Work from an accredited institution.
• Valid and current LCSW license in the state of California.
• Demonstrated experience working with older adults and understanding the complexities related to this demographic.
• Strong clinical and diagnostic skills, with the ability to develop and implement effective treatment plans.
• Excellent communication and interpersonal skills, facilitating positive interactions with clients, families, and colleagues.
• A proven capacity to work both independently and collaboratively as part of a multidisciplinary team.

Preferred Experience

• Experience with telehealth platforms and digital therapy tools to provide remote services effectively.
• Familiarity with community resources and healthcare networks that support older adults.
• Prior experience in a community health clinic or similar setting is a plus.

Compensation and Benefits

We offer a competitive salary ranging from $90,000 to $105,000 annually, commensurate with experience and qualifications. Alongside a strong salary, we provide a comprehensive benefits package that includes:
