This position operates primarily in a home care setting within the community of Appomattox, Virginia. Key Responsibilities

• Conduct comprehensive evaluations of patients in their homes to assess their physical conditions, mobility, and rehabilitation needs.
• Develop and implement individualized treatment plans, utilizing a variety of techniques such as therapeutic exercises, manual therapy, and the use of specialized equipment.
• Monitor patient progress through regular tracking and documentation of treatments, patient responses, and improvements.
• Adjust treatment plans as necessary to meet evolving patient goals and circumstances, ensuring the highest standard of personalized care.
• Educate patients and their families on the use of assistive devices and rehabilitation exercises, fostering a collaborative approach to recovery.
• Provide ongoing training for caregivers and family members to support the recovery process outside of therapy sessions.
• Stay updated on best practices in physical therapy by participating in continuing education opportunities and networking with peers in the profession.

Patient Population / Client Focus

This role focuses on a diverse patient population, including children and adults recovering from various conditions such as post-surgical rehabilitation, orthopedic injuries, neurological disorders, and chronic pain management. Qualifications

• A Bachelor's Degree in Physical Therapy is required; a Doctorate in Physical Therapy (DPT) is preferred.
• A valid physical therapy license in the state of Virginia is mandatory.
• Strong communication skills, both verbal and written, are essential for effective patient interaction and collaboration with healthcare teams.
• Competency in using computer systems for patient documentation and treatment planning.
• Flexibility and agility in adapting to the unique needs of each patient.

Compensation and Benefits
