Advent Care Physical Therapy is actively seeking a dedicated and experienced Physical Therapist to join our team in Appomattox, Virginia. Work Environment

This role is situated in a home care environment where you will visit patients in their residences across Appomattox and surrounding areas. This setting allows you to build personal relationships with patients and engage with them in a familiar and comfortable environment. You will collaborate with other healthcare professionals, including physicians, occupational therapists, and nursing staff, to create a holistic approach to patient care.

Key Responsibilities

• Conduct initial evaluations and ongoing assessments of patients referred by their physicians, ensuring a comprehensive understanding of their physical therapy needs.
• Create individualized treatment plans that align with patient goals and physician recommendations, using evidence-based methodologies.
• Deliver a range of therapeutic interventions directly in patients' homes, which may include therapeutic exercises, manual therapy techniques, and education on home exercises.
• Utilize sophisticated assessment tools to track progress and adjust treatment protocols based on outcomes.
• Demonstrate exceptional customer service skills, fostering a supportive and encouraging atmosphere for patients and their families.
• Collaborate with a multidisciplinary team, sharing insights and strategies to maximize patient outcomes across various care settings.
• Educate patients and their caregivers about rehabilitation processes, home care strategies, and preventative measures to maintain function.
• Meet productivity benchmarks while ensuring high-quality care delivery, maintaining clear, thorough documentation of patient interactions and progress.

Patient Population / Client Focus

As a member of our team, you will work with a diverse patient population, including individuals recovering from orthopedic surgeries, pediatric patients with developmental concerns, neurologic conditions, and sports-related injuries. You will have the opportunity to provide specialized care based on your clinical interests, ensuring each patient receives tailored treatment designed to improve their quality of life.

Qualifications

• Graduation from an APTA or CAPTE-approved physical therapy program.
• Bachelor's degree in Physical Therapy is a requirement; however, a Doctorate of Physical Therapy (DPT) is preferred.
• An active state license for Physical Therapy in Virginia, with the ability to demonstrate relevant continuing education.
• Current Basic Life Support (BLS) certification from the American Heart Association.
• Exceptional verbal and written communication skills, with proficiency in technology and electronic health records.

Preferred Experience

• A minimum of two years of experience in a physical therapy setting, particularly in home care or outpatient facilities.
• Experience working with a variety of patient demographics, including pediatric and geriatric populations.
• Strong organizational skills to manage patient caseload efficiently while maintaining a high standard of care.

Compensation and Benefits

We offer a competitive salary ranging from USD 100,000 to 120,000 annually, in addition to a comprehensive benefits package.
