Key Responsibilities

• Conduct comprehensive evaluations for each patient through detailed history reviews, specific assessments, and the development of individualized treatment plans based on findings.
• Effectively communicate evaluation outcomes and treatment plans to patients and their families, ensuring their involvement in setting achievable and meaningful rehabilitation goals.
• Implement evidence-based treatment protocols tailored to each patient's unique condition and progress, continuously adapting approaches to meet their specific needs.
• Collaborate with a multidisciplinary team of healthcare providers as necessary, recognizing shifts in patient conditions and adjusting care plans accordingly.

Patient Population / Client Focus

Qualifications
