Transformative Journeys Therapy is actively seeking a full-time Psychiatric Nurse Practitioner (PMHNP) to join our dedicated team at our Assisted Living Facility located in beautiful Oyster Bay, New York. Key Responsibilities

• Conduct thorough psychiatric assessments, evaluations, and diagnosis of mental health conditions for residents.
• Manage medication therapy, ensuring optimal effectiveness through regular monitoring and follow-up.
• Collaborate with multidisciplinary teams, including physicians, social workers, and nursing staff, to provide integrated care that encompasses both mental and physical health.
• Implement crisis intervention strategies, offering immediate support to residents experiencing emotional distress.
• Engage in community outreach initiatives aimed at promoting mental health awareness and education.
• Provide therapeutic support through individual and group counseling sessions, as well as psychoeducation, equipping patients with coping strategies and resources.
• Document all patient interactions in compliance with HIPAA regulations, maintaining accurate and confidential records.

Patient Population / Client Focus

This position primarily supports a diverse population within our Assisted Living Facility, serving individuals who may be dealing with various mental health challenges including anxiety, depression, PTSD, and adjustment disorders. You will be tasked with ensuring that care is tailored to the unique cultural and personal backgrounds of each resident, thereby enhancing their comfort and trust in the mental health care process.

Qualifications

• A Master's degree in Nursing with specialization as a Psychiatric Nurse Practitioner.
• Active state licensure as an Advanced Practice Registered Nurse (APRN) with psychiatric specialization in New York.
• Strong communication and interpersonal skills, with an emphasis on empathy and active listening to effectively engage patients and their families.

Preferred Experience

While not mandatory, prior experience working within a community health center or assisted living environment is highly preferred. Familiarity with holistic and culturally sensitive care approaches will be advantageous in aligning with our organizational values.

Compensation and Benefits
