Key Responsibilities

As a Licensed Professional Counselor, your main responsibilities will include:

• Providing therapeutic guidance and instruction to foster the mental, emotional, and physical growth of clients through effective intervention techniques.
• Conducting thorough patient intakes, focusing on diagnostic evaluations to tailor treatment plans that align with each client's specific needs.
• Collaborating closely with primary care providers to identify and manage psychosocial issues, determining the appropriate course of treatment alongside or within the primary care setting.
• Performing follow-up consultations to monitor client progress and adjust treatment plans as necessary.
• Developing relapse prevention strategies and educational materials that empower clients to take charge of their mental health and make informed lifestyle choices.
• Documenting patient's progress, needs, and concerns in compliance with agency and Federally Qualified Health Center (FQHC) guidelines.
• Facilitating effective communication with the primary care team to ensure seamless integration of behavioral health services.
• Conducting consultations with clinic management regarding service effectiveness and program evaluations to continuously enhance care delivery.
• Assisting clients in adhering to prescribed medical treatments, fostering trust and compliance as part of a comprehensive care approach.

Patient Population / Client Focus

Your role as an LPC will primarily focus on a diverse patient population encompassing individuals seeking mental health support, particularly those experiencing stress, anxiety, depression, and other behavioral health challenges. Additionally, you will help identify and assist at-risk individuals who may need specialized care or external referrals, ensuring they receive comprehensive support tailored to their specific circumstances.

Qualifications

The successful candidate will possess:

• A minimum Master’s Degree in Clinical Counseling.
• A current and valid LPC (Licensed Professional Counselor) License.
• In-depth knowledge of mental health assessment methodologies, treatment planning, psychodiagnostics, and various therapeutic techniques.
• Strong understanding of the impact of mental health issues on individuals, families, and the broader community.
• The ability to communicate effectively and maintain cooperative relationships with patients, families, and multidisciplinary teams.
• Proficient record-keeping skills to ensure comprehensive documentation of each client's treatment journey.

Preferred Experience

While not mandatory, candidates with the following qualifications will be highly regarded:

• Bilingual proficiency in English and Spanish, enhancing communication capacity with our diverse client population.
• Experience in a collaborative healthcare setting, particularly within community health clinics or FQHCs.
• A background in developing and implementing educational programs or workshops focused on mental health.

Compensation and Benefits
