Key Responsibilities

• Assess and evaluate patient conditions to develop individualized treatment plans based on their specific needs and recovery goals.
• Utilize diverse therapeutic techniques including hands-on therapy, exercises, and the use of specialized equipment to alleviate pain and improve mobility.
• Regularly document patient progress, responses to treatment, and adjust therapy plans as required to ensure optimal recovery.
• Guide and educate patients, caregivers, and families on therapeutic practices, including the proper use of assistive devices and self-care techniques.
• Stay updated with the latest practices in physical therapy through continuous professional development activities such as workshops and networking with peers.
• Collaborate with the healthcare team, including physicians and nursing staff, to ensure coordinated and comprehensive patient care.

Patient Population / Client Focus

In this position, you will work with a broad spectrum of patients ranging from children to adults, each with unique physical rehabilitation needs. This includes individuals recovering from orthopedic surgeries, stroke patients, and those with chronic conditions that affect mobility. Your role will be critical in helping them regain independence and improve their overall physical well-being.

Qualifications

• A Doctorate Degree in Physical Therapy from an accredited institution.
• Current state license to practice physical therapy.
• Strong written and verbal communication skills to effectively interact with patients and healthcare teams.
• Proficiency in computer and word processing skills for documentation and scheduling.
• Flexibility and adaptability in handling the dynamic nature of home health care environments.

Preferred Experience

• Previous work experience in a physical therapy setting, particularly in home care, is preferred but not required as we are open to mentoring recent graduates.
• Experience with orthopedic rehabilitation practices and protocols is advantageous.

Compensation and Benefits
