The role is based onsite at our rehabilitation facility in San Jose, California, where you will work closely with a multidisciplinary team of healthcare professionals, including physical therapy assistants, occupational therapists, and physicians. Key Responsibilities

• Conduct comprehensive patient evaluations to assess physical function, determine diagnoses, and identify limitations stemming from medical conditions, injuries, or surgical interventions.
• Develop individualized treatment plans based on thorough examinations, incorporating patient goals, age-specific considerations, and relevant medical history.
• Administer a variety of modalities and therapeutic exercises, including manual therapy techniques, electrical stimulation, ultrasound, and therapeutic exercise regimens tailored to each patient's needs.
• Execute treatment plans directly with patients, ensuring safety while using effective therapeutic techniques and maintaining accurate progress notes.
• Adjust treatment approaches based on ongoing assessments of patient response and progress toward goals, providing modifications as appropriate.
• Enhance the patient experience by coordinating care and communicating with other healthcare providers involved in the patient's journey, including involvement in discharge planning.
• Provide patient and family education on injury prevention, rehabilitation strategies, and lifestyle modifications to promote ongoing health and wellness.
• Participate in continuous learning and professional development opportunities, engaging in case management conferences and patient care discussions.

Patient Population / Client Focus

This position will allow you to work predominantly with orthopedic patients, including those recovering from joint replacements, fractures, sports injuries, and chronic pain conditions. Qualifications

• Bachelor's Degree in Physical Therapy or a related field from an accredited program.

• Excellent communication and interpersonal skills, with a commitment to delivering exceptional patient care.

Preferred Experience

Compensation and Benefits
