Work Environment

The Physical Therapist position is designed to be performed onsite, focusing on home settings in the Stafford area. Our therapists work directly within clients' homes, allowing for a personal and tailored approach to therapy administration. Key Responsibilities

• Design and implement customized treatment plans for clients that prevent disabilities, alleviate pain, and enhance muscle function while striving for maximum independence.
• Conduct thorough evaluations using diagnostic tests and functional assessments to gauge the client's physical abilities and environmental adaptations.
• Supervise Physical Therapy Assistants, ensuring the execution of prescribed therapies aligns with the established care plan.
• Administer a variety of physical therapy modalities including manual therapy, therapeutic exercises, traction, and massage techniques.
• Regularly observe, document, and report on the clients' progress, adapting care plans based on changes in condition and collaborating with the client's healthcare team.
• Participate actively in case conferences, contributing insights and recommendations to optimize the treatment plans for our clients.
• Educate clients and their families on physical therapy goals, device usage, and self-management techniques to promote health and recovery.
• Timely documentation of clinical records, including evaluations, care plans, progress notes, and discharge summaries using EMR software, particularly Web PT.
• Conduct site-specific functional assessments and make adjustments to care strategies as required.
• Assist in the development and update of comprehensive care strategies that may involve community resource utilization.

In this role, you will primarily work with patients who require rehabilitation services stemming from various medical conditions, including musculoskeletal disorders, stroke recovery, and post-surgical rehabilitation. Our client population spans diverse demographics, including elderly individuals seeking to maintain independence and those recovering from acute or chronic conditions. Qualifications

• Must have completed a Doctorate in Physical Therapy (DPT) program from an accredited institution.
• Possession of a valid and active Physical Therapist license in the state of Virginia, with a history free from sanctions or board proceedings.
• Expertise in various manual therapy techniques and a proven ability to create effective exercise programs tailored to individual needs.
• Proficient in the use of EMR software, particularly Web PT, ensuring accurate and efficient documentation practices.

Preferred Experience

• A minimum of two years of clinical experience in home healthcare or outpatient physical therapy settings.
