Key Responsibilities\n

• Conduct intake assessments to evaluate patient diagnoses and functional levels, subsequently making informed recommendations to primary care providers.

• Deliver brief, focused therapeutic interventions for patients requiring mental health services, ensuring adherence to best practices.

• Collaborate with primary care providers to determine the appropriateness of continuing care within the primary care setting versus referrals to specialized mental health services.

• Implement and monitor patient follow-ups to assess progress and modify treatment plans as necessary.
\n
• Develop personalized relapse prevention strategies that support long-term recovery and stability for individuals.
\n
• Identify at-risk patients and formulate preventive plans to mitigate deteriorating conditions.
\n
• Document patient progress meticulously, including all diagnostic information, within treatment charts.

• Advise and empower patients with self-management skills and educational resources to encourage active participation in their recovery process.

• Participate in comprehensive team discussions to refine behavioral health services and evaluative measures.

• Assist patients in navigating medical treatments initiated by primary care providers, helping to manage any medication-related side effects.

• Adhere to the ethical standards of clinical practice as articulated in the Code of Ethical Conduct for Clinical Professionals.

Patient Population / Client Focus\n

Your work will primarily focus on patients of all ages within the outpatient setting, addressing various mental health concerns and psychosocial issues. Qualifications\n

• A Master’s Degree in Clinical Counseling is required.

• Valid and current LPC (Licensed Professional Counselor) license in Texas.

Compensation and Benefits\n

We offer a competitive salary range of USD 87,000 to 105,000 per year, commensurate with experience and qualifications.
