Key Responsibilities

• Conduct comprehensive evaluations of patients to understand their medical history, physical condition, and personal goals.
• Develop tailored treatment plans utilizing techniques such as therapeutic exercises, manual therapy, and state-of-the-art equipment to alleviate pain and promote recovery.
• Implement treatments effectively while tracking progress and responding to patient feedback, making adjustments as needed to optimize therapeutic outcomes.
• Educate patients, their families, and caregivers about rehabilitation techniques and the correct use of assistive devices.
• Maintain accurate documentation of patient progress and treatment interventions to ensure quality care and compliance with professionals and regulatory standards.
• Engage in continuous professional development by attending workshops and participating in community outreach to enhance your therapeutic practice.

Patient Population / Client Focus

You will work with a diverse range of patients from children to seniors, each with unique challenges related to their recovery. Whether treating someone recovering from a sports injury, post-surgical rehabilitation, or offering support to individuals with chronic conditions, your role will be pivotal in helping them reclaim their mobility and independence.

Qualifications

• A Doctorate degree in Physical Therapy (DPT) is required.
• Current state license in Physical Therapy.
• Excellent communication skills, both written and verbal, to effectively collaborate with patients and interdisciplinary teams.
• Strong computer skills, with proficiency in healthcare software for documentation and patient management.
• A flexible and proactive approach to developing treatment plans and interacting with patients in a home care environment.
• While a minimum of 2 years of experience is preferred, we welcome new graduates excited to grow their skills with our mentorship program.

Preferred Experience

Experience in home care settings is a plus, but not mandatory. We value a commitment to personal and professional development over specific experiences.

Compensation and Benefits
