Work Environment

Working onsite at our Immokalee Rehabilitation Center, the Physical Therapist will engage in a supportive and interdisciplinary team environment, collaborating closely with healthcare providers, patients, and their families. Key Responsibilities

• Conduct thorough evaluations and assessments of patients with various functional limitations and disabilities to develop individualized treatment plans.
• Provide consultative services to other healthcare practitioners, ensuring continuity of care across different settings.
• Deliver clinical treatment while adhering to customer service standards, maintaining appropriate interactions with patients from all age groups and backgrounds.
• Document findings, treatment plans, and progress accurately in electronic health record systems, ensuring compliance with state regulations and professional standards.
• Effectively communicate with interdisciplinary team members, referral sources, and physicians regarding patient care and progress.
• Participate in department leadership functions such as community outreach, program development, and supervision of support staff.
• Engage in continuous education and personal competency improvement through attending trainings, staff meetings, and in-services.

Patient Population / Client Focus
