Key Responsibilities

• Conduct comprehensive evaluations by reviewing patient histories, performing targeted assessments, and determining the most effective interventions based on evidence-based practices.
• Communicate evaluation findings and the prescribed treatment plan clearly with patients and their families, ensuring they are actively involved in goal setting.
• Demonstrate expertise in implementing tailored treatment protocols and regularly modify these plans according to the patient's progress and evolving healthcare needs.
• Collaborate with other healthcare providers to ensure a holistic approach to patient care, modifying treatment plans when necessary to address changes in patient conditions.
• Oversee discharge planning and follow-up procedures, ensuring accurate documentation of care processes that reflect the patient's progress, goals, and ongoing needs.
• Maintain effective time management, adhere to productivity targets, and ensure accurate scheduling to optimize patient flow and organizational efficiency.

Qualifications

• A Doctorate in Physical Therapy (DPT) is required.
