Work Environment

This position is set in a professional home care environment, where you will provide personalized therapy services directly to patients in their homes. Key Responsibilities

• Conduct comprehensive evaluations to determine individual patient needs, including assessing physical function, mobility, and specific therapeutic goals.
• Develop, implement, and continuously adjust personalized treatment plans based on ongoing patient assessments and progress.
• Utilize a variety of physical therapy techniques, such as manual therapy, therapeutic exercises, and patient education, to facilitate recovery and maintain existing functions.
• Regularly monitor patient progress, documenting outcomes and modifying treatment strategies in response to patient feedback and recovery efforts.
• Collaborate closely with the clinical supervisor and other healthcare professionals to optimize unit activities and deliver an integrated approach to patient care.
• Provide education to patients and their families on exercises, pain management strategies, and lifestyle modifications that can aid in their recovery.
• Participate in team meetings to discuss patient cases and share best practices, promoting a culture of continuous learning and improvement.

Patient Population / Client Focus

The focus of this position is on patients experiencing orthopedic conditions, ranging from post-surgical rehabilitation to chronic pain management and injury recovery. You will work with a diverse range of clients, including elderly patients with age-related mobility issues, athletes recovering from sports injuries, and individuals engaged in rehabilitation after orthopedic surgeries.

Qualifications

• A Bachelor's degree in Physical Therapy is required; a Master's degree (MPT) or Doctorate (DPT) is preferred.
• Active New Jersey state licensure as a Physical Therapist.
• Strong understanding of current physical therapy practices and evidence-based treatment methodologies.
• Excellent interpersonal and communication skills to effectively educate and motivate patients.
• Proficiency in manual therapy techniques and therapeutic interventions tailored to orthopedic rehabilitation.

Preferred Experience

• A minimum of 2 years of experience in a clinical setting, preferably with a focus on rehabilitation and orthopedic care.
• Familiarity with patient management systems and documentation practices in healthcare.
• A proven track record of working collaboratively within a multidisciplinary team to deliver high-quality patient care.

Compensation and Benefits
