Key Responsibilities

• Conduct comprehensive patient evaluations to assess mobility and functional limitations, developing tailored treatment plans that align with patient goals.
• Provide expert consultation to healthcare providers and ensure a seamless continuum of care for patients across all stages of their rehabilitation journey.
• Deliver high-quality patient care by implementing effective treatment protocols, re-evaluating patient progress, and making necessary adjustments based on clinical findings.
• Document all patient interactions, evaluations, treatment plans, and discharge notes in compliance with applicable regulations and standards, ensuring accurate billing and reimbursement processes.
• Engage in interdisciplinary communication, collaborating with physicians, nursing staff, and other therapists to optimize patient outcomes.
• Participate in community outreach initiatives and continuous quality improvement (CQI) efforts that align with our organizational goals, enhancing awareness of physical therapy services.
• Assist in the development and supervision of physical therapy assistants (PTAs), aides, and students, fostering a culture of mentorship and professional growth within the team.
• Remain informed on the latest developments in physical therapy practices through self-study, workshops, and conferences, ensuring compliance with professional standards and licensure requirements.
• Maintain solid relationships with patients, their families, and caregivers, guiding them throughout the treatment and discharge process.
• Proactively adjust treatment approaches based on patient feedback and therapeutic outcomes to optimize care delivery.

Patient Population / Client Focus

Qualifications
