Work Environment\n

This role will be conducted in a remote setting, enabling you to work from the comfort of your home while still being an integral part of a collaborative team. The hospice center focuses on providing compassionate care to patients with life-limiting illnesses and their families, emphasizing the importance of emotional and psychological support in the end-of-life process. You will engage with multidisciplinary teams to ensure that comprehensive and individualized care is delivered.

Key Responsibilities\n

• Conduct detailed evaluations and assessments of patients and families to identify psychosocial needs and determine appropriate interventions.

• Participate actively in the development and implementation of Individualized Family Service Plans (IFSP) tailored to each patient's and family's unique circumstances.
\n
• Monitor and evaluate outcomes as a proactive member of the care team to ensure consistency and effectiveness in the support provided.

• Provide compassionate and informed emotional support to families, addressing any concerns related to their loved one's health and well-being.
\n
• Adhere to all Federal and State regulations regarding social work practice, ensuring that all interventions meet the highest ethical standards.

• Develop rapport with children and families, fostering a compassionate and trusting relationship that promotes a healthy working environment.
\n
• Implement evidence-based practices endorsed by leading health organizations, utilizing a coaching model and natural environment teaching.
\n
• Facilitate family education sessions to empower caregivers with skills and information to enhance their child's development.
\n
• Collaborate with healthcare professionals to ensure integrated care pathways, effectively bridging emotional and medical needs.

Patient Population / Client Focus\n

This role primarily involves working with children under three years old, especially those with disabilities or developmental delays. As an LCSW, your expertise will guide families through the complexities of their child's needs, offering strategies and interventions designed to foster growth and learning in a supportive environment.

Qualifications\n

• A Master’s Degree or Doctorate in Social Work is required.

• Current licensure as a Licensed Clinical Social Worker (LCSW) by the Department of Public Health.
