Key Responsibilities

• Conduct comprehensive mental health assessments, integrating insights related to physical health to provide a thorough understanding of each patient's situation.
• Develop and implement individualized treatment plans that address both mental health and co-occurring medical conditions, incorporating evidence-based practices to promote patient success.
• Facilitate individual and group therapy sessions adapted to the specific needs of patients, using therapeutic techniques that support their personal growth and emotional resilience.
• Collaborate with healthcare providers to ensure a cohesive approach to patient care, contributing to improved treatment outcomes and overall health management.
• Provide crisis intervention services, responding promptly to mental health emergencies with sensitivity and expertise.
• Educate patients about the connection between mental health and physical health to empower them in managing their conditions effectively.
• Ensure meticulous maintenance of patient records and documentation, adhering to both legal and ethical standards to protect patient confidentiality and maintain compliance.

• Stay informed about the latest research and best practices in integrated care to continually improve service delivery and patient outcomes.

Patient Population / Client Focus

This role primarily focuses on individuals with complex mental health and co-occurring medical conditions, including but not limited to chronic illnesses, trauma histories, and socio-economic challenges. Counselors will work with a diverse patient demographic within the Fresno community, ensuring that care strategies are culturally responsive and sensitive to individual patient backgrounds.

Qualifications

• A Master’s or Doctoral degree in Social Work, Marriage and Family Therapy, Mental Health Counseling, or Psychology from an accredited institution.
• Active and valid licensure as an LCSW, LPCC, or LMFT in California.
• Demonstrated experience providing mental health services to individuals with co-occurring medical conditions.
• Excellent communication and interpersonal skills, with a capacity for building rapport with patients and colleagues alike.
• Strong adherence to ethical guidelines and professional boundaries while ensuring patient confidentiality.
• Proficiency in utilizing electronic health record (EHR) systems and related technology to document care.
• A collaborative mindset, with the ability to work seamlessly as part of an interdisciplinary care team.

Preferred Experience

While the required qualifications include a relevant master’s degree and state licensure, preferred candidates will have demonstrable experience in integrated care settings, specifically dealing with patients who present with multiple and complex health needs. Familiarity with teletherapy platforms and experience working in remote or community health clinics will be advantageous.

Compensation and Benefits

The salary range for this position is competitive, set between $115,000 and $130,000 annually. Additional benefits include:
