This role is designed to be performed remotely, allowing you the flexibility to balance professional and personal commitments. While the majority of your work will take place from your home or another suitable location, you will also have occasional opportunities to participate in in-person meetings at our outpatient office in Dallas, Texas. Key Responsibilities\n

• Perform comprehensive assessments to identify the mental health needs of patients, ensuring continuity of care through regular evaluations.

• Maintain open lines of communication with family members, caregivers, and relevant professionals about each patient’s progress, ensuring comprehensive support.

• Develop and provide a detailed schedule of appointments, home visits, and monitoring activities to supervisors, enhancing treatment adherence and accountability.

• Coordinate and conduct essential home visits for in-home observations, which may include chart reviews and interviews in order to inform therapy strategies.

• Oversee the implementation of treatment plans, ensuring that interventions are effective and properly executed.

• Participate actively in interdisciplinary team meetings and other required gatherings to discuss case progress and treatment innovations.

• Create and update monthly and quarterly progress notes that reflect patient advancements and challenges.

• Lead training sessions for support staff and mental health team members on the effective implementation of program plans and documentation standards.

• Conduct follow-up interviews and observations to ensure ongoing support for individual therapy goals.

• Manage accurate and timely documentation to make necessary information easily accessible to authorized personnel.
