Key Responsibilities

• Conduct comprehensive mental health assessments that consider the impact of co-occurring medical conditions on patient well-being.
• Develop personalized, integrated treatment plans that address both psychiatric and medical needs of patients.
• Facilitate both individual and group therapy sessions that are grounded in evidence-based practices, adapting approaches to meet individual patient circumstances effectively.
• Collaborate with medical providers to ensure a coordinated approach to patient care, enhancing treatment outcomes across disciplines.
• Provide crisis intervention and immediate support during mental health emergencies, ensuring patient safety and resource access.
• Educate patients on the relationship between mental health and physical well-being, empowering them to take an active role in their care.
• Maintain accurate, confidential records in compliance with legal, ethical, and organizational standards.
• Engage in continuous professional development through participation in team discussions, staying informed of best practices and advancements in integrated care.

Patient Population / Client Focus

Your work will focus on a diverse client population, including individuals managing co-occurring medical conditions such as autoimmune disorders, substance use disorders, and chronic pain. The complexity of their needs demands both compassion and expertise in navigating the intersection of mental and physical health.

Qualifications

• A Master’s or Doctoral degree in Social Work, Marriage and Family Therapy, Mental Health Counseling, or Psychology from an accredited institution.
• Current licensure as an LCSW, LPCC, or LMFT in the state of California.
• Proven experience in addressing mental health challenges in clients with co-occurring medical conditions.
• Exceptional interpersonal and communication abilities to foster strong therapeutic relationships.
• Commitment to upholding professional boundaries and maintaining confidentiality in all interactions.
• Familiarity with electronic health record (EHR) systems and telehealth technologies.
• A collaborative spirit with a strong inclination to work within an interdisciplinary team.

Preferred Experience

• Prior experience in a telehealth setting is advantageous but not mandatory.
• Knowledge of treatment modalities specific to chronic illness management.
• Experience in crisis intervention and emergency response protocols.

Compensation and Benefits

The salary for this position ranges from USD 115,000 to 130,000 annually, commensurate with experience and qualifications. Additionally, we offer a comprehensive benefits package that includes:
