Work Environment

Key Responsibilities

As a Full-Time Home Care Physical Therapist, your responsibilities will include:

• Designing and implementing individualized physical therapy treatment plans aimed at preventing disability, alleviating pain, and enhancing muscle function.
• Overseeing and guiding physical therapy assistants to ensure effective execution of care plans.
• Evaluating clients through comprehensive assessments encompassing diagnostic and prognostic tests to ascertain their functional abilities.
• Conducting environmental assessments to modify clients' home settings to promote rehabilitation and independence.
• Administering therapeutic exercises and engaging clients in functional activities to promote improvement.
• Applying various physical therapy modalities, including traction and massage, to relieve pain and enhance recovery.
• Assessing, fitting, and recommending adjustments to assistive devices tailored to client needs.
• Documenting all clinical interactions and maintaining accurate records, including evaluations, care plans, and discharge summaries.
• Participating in multidisciplinary team meetings to collectively evaluate and strategize client care.
• Educating clients and their families about rehabilitation processes and the use of physical therapy tools to encourage active participation in their recovery.

Patient Population / Client Focus

You will predominately work with a diverse range of patients, including those recovering from surgery, managing chronic pain, or dealing with mobility challenges due to aging or disability. Your approach will be individualized, addressing each client's unique circumstances, preferences, and rehabilitation goals. Ensuring a positive experience for clients and their families is central to our practice.

Qualifications

Ideal candidates will possess the following qualifications:

• A Bachelor's degree from an accredited physical therapy program.
• At least two years of proven experience as a physical therapist, preferably in home care or acute care settings.
• Current licensure to practice physical therapy in Florida, in good standing.
• CPR certification current and valid.
• A valid driver's license and proof of automobile insurance.
• A dedication to ongoing clinical excellence and a passion for improving patient outcomes.
• Strong interpersonal skills with the ability to communicate effectively with patients, families, and colleagues.

Preferred Experience

A proven background in home health services, particularly in developing care plans and working collaboratively with a healthcare team, will give candidates an edge in this dynamic role. Familiarity with relevant regulations and exceptional documentation practices are also preferred.
