Work Environment

Key Responsibilities

• Conduct comprehensive evaluations of patients, gathering pertinent histories, performing specific assessments, and documenting results to develop effective treatment plans.
• Engage with patients and their families to explain evaluation findings and collaboratively set attainable goals.
• Implement tailored treatment protocols according to each patient's unique needs, continually adapting based on progress and changes in condition.
• Communicate and consult with various healthcare professionals to ensure cohesive treatment plans and recognize any necessary changes in patient care.
• Complete discharge planning, following up on patient care to ensure continuous support as they transition out of rehab.
• Document patient care activities accurately and in a timely manner, maintaining thorough records that detail initial evaluations, treatment plans, and progress notes.
• Meet defined productivity expectations while managing your schedule effectively and participating in community outreach to promote our services.
• Foster a culture of continuous learning by seeking out professional development opportunities and mentoring students in their clinical education.

Patient Population / Client Focus

Qualifications

• A Doctorate in Physical Therapy, along with a Bachelor's degree from an accredited physical therapy program.

Compensation and Benefits
