Immediate Pain Management Therapy is seeking a dedicated Home Care Orthopedic Physical Therapist to join our team in beautiful Punta Gorda, Florida. Work Environment

This position will primarily be conducted onsite in patients' homes across Punta Gorda and surrounding areas. Working in a home care setting enables therapists to build closer relationships with patients and deliver personalized therapy in a familiar, comfortable environment.

Key Responsibilities

• Evaluate and treat patients recovering from injuries, surgeries, or ongoing health issues in their homes, utilizing evidence-based practices to optimize outcomes.
• Collaborate with patients to establish comprehensive, individualized treatment plans that incorporate exercise, manual therapy, and the use of necessary equipment to alleviate pain and promote mobility.
• Monitor patients' progress, adjusting treatment plans as necessary based on evaluations and feedback to ensure therapeutic goals are achieved.
• Document treatment sessions, patient responses, and progress in electronic health records, maintaining high standards for accuracy and compliance.
• Provide training to patients, family members, and caregivers on therapeutic exercises and equipment usage, ensuring they feel empowered in the recovery process.
• Maintain current knowledge through continuing education opportunities, attending workshops, and engaging with professional organizations to enhance clinical skills.

Patient Population / Client Focus

As a Home Care Orthopedic Physical Therapist, you will work with a diverse patient demographic, including children and adults recovering from various orthopedic conditions such as fractures, joint replacements, and sports injuries. Qualifications

• A Bachelor's Degree in Physical Therapy is required, with preference given to candidates with a Doctorate in Physical Therapy.
• Active State License in Physical Therapy.
