Key Responsibilities

As a Home Care Physical Therapist, your primary responsibilities will include:

• Developing and implementing personalized treatment plans based on physician prescriptions, tailored to the unique needs and goals of each client.
• Directing the provision of physical therapy services to prevent disability, relieve pain, and restore function through targeted exercises and therapeutic modalities.
• Evaluating clients' functional abilities utilizing objective tests to determine their progress and necessary adjustments to their care plans.
• Conducting thorough assessments of clients' home environments to recommend modifications that enhance their independence and safety.
• Collaborating closely with physical therapy assistants, ensuring effective and appropriate delivery of therapy services while supervising their interventions in compliance with state laws.
• Continuously monitoring clients' progress, documenting observations, and communicating updates with physicians and interdisciplinary team members.
• Preparing and submitting accurate clinical records, including initial evaluations, progress notes, and discharge summaries.
• Engaging in interdisciplinary case conferences to formulate comprehensive treatment plans that address both medical and personal care needs.

Patient Population / Client Focus

Qualifications

To succeed in this role, candidates are required to have:

• A Doctorate in Physical Therapy (DPT) from an accredited program.
• A valid Physical Therapy license in Virginia, with a commitment to maintaining good standing.
• Current CPR certification and a valid driver's license with automobile insurance.
• A strong foundational knowledge of physical therapy best practices and regulations.

Preferred Experience

Ideal candidates will also have:

• A minimum of two years of clinical experience as a physical therapist, preferably in an acute care or home care setting.
• Expertise in assessing and implementing care for diverse patient populations, including those with complex cases requiring interdisciplinary approaches.
• Demonstrated ability to sequentially delegate tasks while supervising staff effectively.

Compensation and Benefits
