Key Responsibilities

• Collaborate with a comprehensive therapy and facility team to devise and execute tailored treatment plans that align with each patient's unique needs and conditions.
• Conduct thorough evaluations of patients' physical abilities and establish baseline metrics to track progress throughout the recovery journey.
• Educate patients and their families about the importance of physical therapy, empowering them to take an active role in the rehabilitation process.
• Lead and participate in regular interdisciplinary Care Plan Meetings to discuss patient progress, share insights, and adjust treatment plans as necessary.
• Provide in-service education sessions to facility staff, raising awareness of effective therapy techniques and improving overall patient care.
• Maintain accurate and timely documentation of patient assessments, treatment plans, progress notes, and discharge summaries in accordance with clinic policies.
• Utilize therapeutic modalities and rehabilitation equipment to assist patients in improving mobility, strength, and overall functionality.

Patient Population / Client Focus

The ideal candidate will work within a skilled nursing facility, focusing on an elderly patient population with diverse rehabilitation needs, including post-operative recovery, fall prevention, and chronic pain management. Your ability to foster relationships with patients and provide empathetic care will be essential in guiding them toward successful recovery outcomes.

Qualifications

• A valid Physical Therapist license in good standing in Virginia.
• Current CPR certification.
• Completion of a Doctorate in Physical Therapy.
• Strong commitment to professional development and staying updated with the latest evidence-based practices in physical therapy.
• Excellent communication skills to build rapport with patients, families, and multidisciplinary team members.

Preferred Experience

Compensation and Benefits
