Work Environment

This role is based onsite, primarily within home care settings. You will independently visit patients in their residences, collaborating closely with their families and other healthcare professionals to deliver effective treatment tailored to their unique circumstances. Our facility promotes a supportive and dynamic work culture, ensuring that you have the resources and support needed to succeed in your role. Expect a structured yet flexible work schedule with standard hours from Monday to Friday, 9:00 AM to 5:00 PM.

Key Responsibilities

• Conduct thorough assessments of patients' functional abilities and limitations to create personalized treatment plans aimed at achieving their rehabilitation goals.
• Utilize a wide range of therapeutic techniques and modalities, such as manual therapy, therapeutic exercises, and modalities aimed at pain relief and mobility enhancement.
• Educate patients and their families on safe movement techniques, exercise regimens, and strategies to maintain long-term wellness and prevent re-injury.
• Regularly monitor and document patient progress, modifying care plans as needed based on objectives and outcomes.
• Design and implement specialized fitness and wellness programs that cater to the individual needs and conditions of each patient.
• Promote healthy lifestyle choices through strength training, flexibility exercises, balance enhancement, and coordination improvement.

Patient Population / Client Focus

The primary focus of this role will be working with patients recovering from orthopedic injuries, surgeries, and chronic musculoskeletal conditions. You will encounter diverse individuals from various backgrounds, requiring a compassionate and adaptable approach to treatment. Your expertise will directly contribute to their rehabilitation journey, ensuring they regain mobility, functionality, and confidence in their daily lives.

Qualifications

• A Master's degree or Doctorate in Physical Therapy from an accredited institution.
• An active State license for Physical Therapy, demonstrating your commitment to maintaining professional standards.
• A minimum of two years of hands-on experience in orthopedic physical therapy, showcasing your ability to work effectively with diverse patient populations.
• Excellent communication skills, both verbal and written, enabling you to interact professionally with patients and healthcare team members.
• Strong organizational skills to manage patient caseloads effectively and document care actions thoroughly.

Preferred Experience

• Experience in home health settings, providing care in patients' homes, which enhances your adaptability to different environments and patient needs.
• Proficiency in the latest evidence-based treatment techniques and interventions, ensuring that your practice is efficient and current.
• Familiarity with electronic health record (EHR) systems for documentation and patient management.

Compensation and Benefits

We offer an attractive salary range of USD 110,000 to 130,000 annually, commensurate with experience and qualifications.
