Work Environment

This role is performed onsite, directly within patients' homes. Working in a home care setting allows our therapists to create personalized, intimate connections with clients, ensuring comprehensive assessments and effective intervention tailored to individual needs. You will collaborate with a multidisciplinary team to facilitate the best possible care and recovery experiences.

Key Responsibilities

• Conduct thorough evaluations of patients referred by physicians, developing and implementing tailored treatment plans based on individual assessments.
• Utilize diverse treatment modalities and therapeutic interventions, addressing a range of conditions and disabilities including orthopedic injuries, neurologic disorders, pediatric concerns, and sports-related injuries.
• Provide ongoing education and support to patients and their caregivers, empowering them with the knowledge and strategies necessary to continue progress outside of therapy sessions.
• Document treatment plans and progress notes meticulously to ensure continuity of care and effective communication within the healthcare team.
• Demonstrate exceptional customer service and professional conduct, contributing positively to the overall therapeutic environment.
• Meet productivity goals and participate in business development efforts, maintaining a strong focus on patient satisfaction and successful outcomes.

Patient Population / Client Focus

As a Home Care Physical Therapist with Advent Care, you will primarily serve patients who are recovering from various medical conditions or surgeries, athletes seeking to enhance their performance, and individuals with chronic issues requiring ongoing management and rehabilitation. Your work will greatly impact the quality of life for these clients by helping them regain mobility, strength, and independence.

Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited program is required.
• Possession of a current state license as a Physical Therapist for Virginia.
• Certification in Basic Life Support (BLS) is mandatory.
• Strong communication skills, both verbal and written, are essential for effective patient interaction and documentation.
• Proficient in using basic computer applications and electronic health record systems.
• A commitment to foster meaningful relationships with patients, helping them achieve their treatment goals.

Preferred Experience

• A Doctorate of Physical Therapy (DPT) from a CAPTE-accredited program is preferred, with an active license as a DPT in Virginia.
• At least two years of clinical experience in physical therapy, particularly in a home care setting or outpatient environment, is desirable.
• Active involvement in community outreach or engagement initiatives to promote physical wellness is a plus.

Compensation and Benefits

The salary range for this role is competitive, between USD 100,000 and 120,000 annually, commensurate with experience and qualifications. In addition to a strong salary, we offer a comprehensive benefits package that includes:
