Key Responsibilities\n

• Conduct holistic and ongoing assessments to evaluate the mental health needs of patients and tailor individualized care plans accordingly.

• Facilitate clear and consistent communication with supervisors, fellow care providers, and family members to ensure that each patient’s progress is comprehensively monitored.

• Develop and propose structured schedules for patient appointments, home visits, and follow-up assessments, ensuring efficient time management and adherence to program objectives.

• Coordinate home visits for in-home evaluations and support, delivering quality mental health care that aligns with patient needs and fostering trust and safety.

• Implement program plans effectively to meet organizational standards, continuously evaluating and refining strategies to enhance patient outcomes.

• Actively participate in interdisciplinary team meetings and contribute to collaborative discussions aimed at improving service delivery.

• Document monthly and quarterly progress reports that capture client progress and facilitate continuous improvement of services.

• Organize and lead training sessions to ensure that staff members are adequately trained in program implementation and understand documentation requirements.

• Provide follow-up support that includes interviews, home visits, and observations to identify ongoing needs and adjust care as necessary.

• Maintain comprehensive records to ensure transparency and accessibility of information among team members and stakeholders.

• Serve as a liaison within the mental health community, nurturing relationships that support community goals for mental wellness.

• Contribute to the development of procedural training materials aimed at enhancing the quality of programs offered to clients.

Patient Population / Client Focus\n

You will engage with a diverse range of clients, including children, adolescents, and adults, offering therapy focused on individual, couples, and family dynamics. Qualifications\n

• A Master's or Doctoral degree in Counseling, Psychology, or a related field is required.

• Active and unrestricted Professional Counselor (LPC) licensure in Texas is mandatory.

• Minimum of 2 years of counseling experience in a clinical context.

• Strong preference for applicants with specialization in family therapy, couples therapy, trauma, or substance use disorders.

• Comprehensive understanding of legal and ethical standards governing mental health practice.

• Exceptional interpersonal and communication skills to build rapport with diverse client populations.
