You will engage directly with patients and work alongside a skilled interdisciplinary team of healthcare professionals, ensuring a comprehensive approach to treatment and recovery.

Develop and implement individualized physical therapy treatment plans aimed at preventing disability, alleviating pain, and restoring muscle function according to each patient's unique capabilities.

Oversee and collaborate with Physical Therapy Assistants to ensure the execution of the prescribed plan of care, while providing guidance and support.

Provide in-home physical therapy services, delivering therapy as prescribed by physicians, while also engaging in manual therapy techniques and functional exercises.

Monitor and document changes in patient conditions and responses to therapy, making appropriate recommendations to revise treatment plans when necessary.

Participate actively in interdisciplinary team meetings, sharing insights and collaborating on holistic care approaches to treatment.

Instruct clients and their families on the effective use and care of physical therapy devices, promoting education around the entire physical therapy process.

Prepare and maintain accurate clinical records, including initial evaluations, progress notes, and discharge summaries to ensure continuity of care.

Stay accessible to Physical Therapy Assistants and provide support during treatment sessions as required.

The physical therapist will work with a diverse array of patients, ranging from post-operative individuals to those seeking rehabilitation for chronic conditions. The primary focus is on elevating patients' quality of life through personalized treatment approaches and therapeutic interventions that cater to their specific recovery goals. A Doctorate in Physical Therapy (DPT) from an accredited program.

An active Physical Therapist license in New Jersey is mandatory, with an exemplary record void of sanctions or board proceedings.

Proficient in using electronic medical records (EMR) software, particularly Web PT, for accurate documentation and maintaining patient records.

Up-to-date knowledge of modern physical therapy techniques and practices, supported by current research and best practices.

Experience in a rehabilitation facility or home healthcare setting is advantageous.

Skills in teaching or mentoring Physical Therapy Assistants can be beneficial for teamwork efficiencies.
