Perform thorough evaluations of patients' physical health by reviewing medical history, conducting assessments, and documenting findings.

Implement individualized treatment protocols based on patient assessments and adapt as necessary to respond to changing health conditions.

Collaborate with other healthcare professionals to ensure comprehensive care and timely adjustments to treatment plans.

Manage discharge planning and follow-up care, meticulously documenting patient interactions and treatment progress to ensure continuity of care.

Contribute to department initiatives, participating actively in marketing efforts to enhance community engagement and awareness of services.

Stay current with best practices through ongoing education and training, enhancing both personal growth and overall team competency.

Foster excellent customer experiences by adapting services to meet the diverse needs of patients and their families.

As a home care Physical Therapist, you will work primarily with patients recovering from surgery, injury, or illness who require in-home rehabilitation. A Bachelor's degree in Physical Therapy from an accredited institution.

A valid state license to practice Physical Therapy in Florida.

American Heart Association's BLS for Healthcare Providers certification is mandatory.

A High School Diploma or GED equivalent.

While prior work experience is not required, a passion for patient care and a commitment to professional development are essential.

While no prior experience is required, candidates with background knowledge of home health care practices, as well as familiarity with electronic health record (EHR) systems, will find themselves well-equipped for the unique challenges of this role.
