Key Responsibilities\n

• Perform comprehensive assessments to evaluate the needs of patients, developing specific treatment plans tailored to individuals and their circumstances.

• Maintain effective communication with supervisors, direct care staff, families, and other relevant parties to monitor patient progress and adjust treatment plans as necessary.

• Create and provide a structured schedule for appointments, home visits, and teletherapy sessions to ensure continuous support and accountability for clients.

• Coordinate and document home visits when appropriate, gathering critical data through chart reviews and staff interviews to enhance service quality.

• Participate actively in Interdisciplinary Team Meetings to contribute insights and collaborate on integrated care strategies.

• Maintain accurate and timely progress notes, ensuring compliance with legal and ethical standards governing mental health practice.

• Organize training sessions to equip staff members with the knowledge necessary to implement effective treatment plans and documentation practices.

• Actively engage with the community and external resources to build a supportive network and advocate for client needs.

• Continuously gather and analyze data to improve organizational practices and patient outcomes.

In this role, you will serve a diverse range of clients, including both youth and adults, facing various mental health challenges such as anxiety, depression, trauma, and substance misuse. Qualifications\n

• A Master's or Doctoral degree in Counseling, Psychology, or a closely related field.

• Active and unrestricted licensure as a Clinical Professional Counselor (LCPC) in the state of Illinois.

• A minimum of 2 years of experience in counseling within a clinical or telehealth context is required.

• Strong foundation in legal and ethical standards pertinent to mental health practice.

• Exceptional interpersonal and communication skills to establish rapport with clients and families.

• Demonstrated ability to collaborate effectively with a multidisciplinary healthcare team.
