Key Responsibilities

• Conduct comprehensive evaluations for patients, reviewing their medical histories, conducting specific assessments, and documenting the results to craft tailored treatment plans.
• Clearly explain evaluation findings and treatment approaches to patients and their families, ensuring they are actively involved in the goal-setting process.
• Implement treatment protocols based on individual evaluation findings while adapting to the evolving healthcare landscape and patient needs.
• Consult with interdisciplinary teams when necessary, accurately tracking and documenting any changes in patient conditions that may require modifications to their care plans.
• Manage discharge planning, provide follow-up care instructions, and maintain precise documentation for patient care activities, including initial evaluations, ongoing progress, and final discharge summaries.
• Demonstrate effective time management skills to meet departmental productivity expectations while attending scheduled meetings punctually.
• Actively participate in community outreach and marketing initiatives to promote the clinic's services and enhance community engagement.
• Contribute to departmental objectives, complying with regulations, and taking the initiative to pursue continuing education for professional development.
