Position Overview

Access Control Physical Therapy & Wellness is actively seeking a seasoned Physical Therapist (PT) to join our dynamic team at our rehabilitation center located in Freehold, New Jersey. This full-time position offers an opportunity for a passionate and dedicated therapist to provide exceptional care while earning an annual salary in the range of USD 100,000 to 120,000. Work Environment

This position is located onsite at our state-of-the-art rehabilitation center in Freehold, New Jersey. You will be part of a collaborative team of healthcare professionals, working in an environment that emphasizes teamwork and mutual support. Our facility is equipped with advanced rehabilitation technologies and therapeutic modalities to effectively address the needs of our clients. The standard working hours for this role are from 8:00 am to 5:00 pm, Monday through Friday.

Key Responsibilities

As a Physical Therapist at Access Control Physical Therapy & Wellness, you will be responsible for:

• Conducting thorough evaluations of patients presenting with functional limitations and disabilities and formulating individualized treatment plans based on evidence-based practices.
• Delivering proficient evaluation services, which include gathering relevant medical history, performing specialized assessments, and documenting comprehensive results and recommendations.
• Implementing tailored treatment protocols grounded in the initial evaluation findings, while adapting techniques to meet evolving patient needs and progress.
• Collaborating with other healthcare professionals to optimize care plans, recognizing critical changes in patient conditions that may necessitate adjustments in treatment or additional consultations.
• Executing meticulous discharge planning, ensuring seamless follow-up care for patients while maintaining comprehensive and accurate documentation of all patient interactions, treatment sessions, and progress notes.
• Staying compliant with all state, local, and federal regulations, while completing necessary corporate and clinical training programs.
• Evaluating, treating, and directing the care for patients within your licensure, ensuring clarity in medical necessity for therapy services.
• Engaging in facility meetings and contributing to the continual improvement initiatives aimed at enhancing patient access and outcomes.

Patient Population / Client Focus

The patients you will serve primarily include individuals requiring orthopedic rehabilitation due to a variety of conditions such as post-surgical recovery, sports injuries, or chronic pain issues. Your role is pivotal in helping these patients regain their functional independence and improve their quality of life, illustrating the real impact of skilled physical therapy on health outcomes.

Qualifications

To be considered for this position, candidates must possess:

• A Doctorate of Physical Therapy (DPT) from an accredited physical therapy program.
• Current state licensure as a DPT in New Jersey.
• At least two years of experience in outpatient orthopedic settings.
• A valid CPR certification.

Preferred Experience

While not mandatory, having experience with specialized therapeutic techniques or certifications in areas such as manual therapy or sports rehabilitation would be beneficial in enhancing your practice. Familiarity with electronic health record (EHR) systems is also highly regarded.
