Key Responsibilities\n

As a Licensed Mental Health Counselor at Evergreen, your responsibilities will include:

• Conducting in-depth mental health assessments, taking into account the interplay between mental health and any co-occurring medical conditions such as autoimmune disorders or chronic pain.

• Developing and implementing comprehensive treatment plans that encompass both psychiatric and medical considerations, tailored to each patient’s unique situation.

• Providing individual and group therapy sessions utilizing evidence-based practices that foster personal growth and resilience among clients.

• Collaborating with a multidisciplinary team of medical providers and specialists to ensure cohesive care and optimize treatment outcomes for patients.

• Offering crisis intervention and management support during mental health emergencies, maintaining a calm and supportive environment for clients in distress.

• Educating patients about the relationship between their mental health and physical conditions, promoting self-management techniques and strategies.

• Maintaining accurate and confidential documentation, adhering to all legal and ethical standards associated with patient care

Patient Population / Client Focus\n

You will work primarily with a diverse patient population, including individuals facing complex challenges such as mental health disorders along with chronic physical health conditions. Your role is pivotal in providing empathetic support and evidence-based interventions that address both aspects of wellbeing, helping clients navigate their personal journeys toward improved health.

Qualifications\n

To be successful in this role, candidates must possess the following qualifications:

• A Master’s or Doctoral degree in Social Work, Marriage and Family Therapy, Mental Health Counseling, or Psychology from an accredited institution.

• Current and valid licensure as an LCSW, LCSW-R, LMHC, or LMFT in the state of New York.

• Proven experience addressing mental health needs for individuals with co-occurring medical conditions.

• Strong interpersonal and communication skills, with the ability to build rapport with clients and collaborate effectively with teams.

• Capacity to maintain professional boundaries and utmost confidentiality while ensuring adherence to ethical guidelines.

• Proficiency in using electronic health record systems and other computer applications relevant to mental health practice.

Preferred Experience\n

While not mandatory, experience in a community health setting and familiarity with integrated care models are advantageous. Being adept in utilizing trauma-informed care approaches and having experience in crisis intervention will enhance your effectiveness in this role.
