Key Responsibilities\n

• Conduct comprehensive assessments to accurately determine the mental health needs of patients.

• Communicate effectively with supervisors, direct care staff, family members, and guardians to optimize patient care and coordinate efforts regarding individual progress.

• Maintain a structured schedule for appointments, home visits, and monitoring activities, while ensuring compliance with care plans.
\n
• Organize and oversee home visits, including in-home observations and staff interviews, to facilitate data collection and analysis.
\n
• Actively participate in Interdisciplinary Team Meetings and contribute to collaborative care initiatives.
\n
• Document patient progress through monthly and quarterly reports to ensure continuity of care.
\n
• Lead training sessions to enhance team capabilities in implementing programs and maintaining accurate documentation.
\n
• Provide ongoing follow-up through interviews and home visits to ensure patient needs are adequately met.
\n
• Serve as a liaison between patients and the wider mental health community, advocating for resources and support.
\n
• Develop and refine training materials and procedures designed to improve service delivery and support for individuals in care.

Patient Population / Client Focus\n

The LPC will work with a wide variety of client populations, including both youths and adults facing diverse mental health challenges. This may encompass issues related to family dynamics, substance use, trauma, anxiety, and depression. Your role will not only involve individual counseling sessions but also the potential to provide support in larger family and community contexts.

Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, Social Work, or a related field.

• Current and unrestricted licensure as a Professional Counselor (LPC) in the state of Texas.
\n
• A minimum of 2 years of relevant counseling experience, ideally within a clinical setting.

• Strong understanding of legal and ethical standards governing mental health practice.
\n
• Exceptional interpersonal and communication skills that enable rapport-building with clients and stakeholders.

• Demonstrated ability to work collaboratively within a healthcare team.

• Familiarity with Electronic Medical Records (EMR) systems and related technologies.
