Work Environment

This position offers an onsite work environment, primarily delivering therapeutic services in patients' homes. The role involves a standard 40-hour work week, with variable weekend hours to accommodate patient needs. Our interdisciplinary team collaborates closely to support patient care and outcomes, making a significant difference in the recovery and quality of life for those we serve.

Key Responsibilities

As a Physical Therapist, you will undertake various responsibilities that include:

• Examining and evaluating patients to assess functional limitations and disabilities.
• Developing individualized treatment plans in collaboration with patients and their families based on comprehensive assessments.
• Implementing therapeutic interventions and continuously re-evaluating patient progress to ensure optimal outcomes.
• Consulting with other healthcare providers regarding patients' ongoing care and adjustments to treatment plans as necessary.
• Documenting patient treatment progress and providing necessary reports to physicians at key intervals, including evaluations at admission and discharge.
• Participating in community outreach and education initiatives to promote health and wellness.
• Mentoring and supervising support staff, promoting a cohesive team environment.
• Engaging in continual professional development through attending workshops, conferences, and in-house training programs.

Patient Population / Client Focus

Our service model focuses on a diverse patient population requiring home care therapy due to orthopedic conditions, post-surgical recovery, or chronic illness. You will be working with patients of varying ages and cultural backgrounds, delivering tailored interventions that honor their unique needs and circumstances. Your ability to communicate effectively and empathetically will be essential in building trust and rapport with patients and their families.

Qualifications

The ideal candidate will possess the following qualifications:

• A Bachelor's Degree in Physical Therapy from an accredited institution.
• Current state licensure in Physical Therapy.
• Basic Life Support (BLS) certification.
• Strong verbal and written communication skills.
• Proficiency in electronic health record systems and word processing software.

Preferred Experience

While a minimum of two years of experience in a physical therapy setting is required, prior experience in home health care or post-acute rehabilitation will be considered a significant advantage. A proactive approach to patient engagement and familiarity with evidence-based practice models will highly contribute to your success in this role.
