Key Responsibilities

• Conduct comprehensive evaluations of patients in home care settings to assess physical therapy needs and determine effective treatment plans.
• Implement personalized treatment protocols utilizing a variety of methods, such as tailored exercise regimens, manual therapy, stretching techniques, and therapeutic modalities.
• Regularly document patient progress and modify treatment plans based on ongoing assessments and changing patient needs.
• Educate patients, families, and caregivers about self-management strategies and therapeutic exercises to promote independent living and enhance mobility.
• Provide guidance on selecting and fitting appropriate assistive devices to improve patients' daily functioning and safety.
• Engage in professional development activities to remain current with industry advancements and improve therapeutic practices.
• Collaborate with a multidisciplinary team of healthcare professionals to ensure holistic care and optimal patient outcomes.

Patient Population / Client Focus

The ideal candidate will work with a diverse patient population, including children and adults recovering from orthopedic injuries, surgeries, and chronic conditions. Qualifications

• A Doctorate degree in Physical Therapy (DPT) from an accredited institution.
• A valid State License in Physical Therapy to ensure compliance with local regulations and standards.

• Proficient in basic computer skills for documentation and administrative duties.

Preferred Experience
