Key Responsibilities

• Conduct thorough patient evaluations by reviewing medical histories, performing assessments, and documenting findings to formulate individualized treatment plans.
• Effectively communicate evaluation results and treatment strategies to patients and their families, ensuring their involvement in goal-setting to enhance adherence and motivation.
• Implement evidence-based treatment protocols appropriate for each patient's specific needs, adjusting as necessary based on their progress and health changes.
• Collaborate with a multidisciplinary team, consulting with doctors, nurses, and other healthcare practitioners to coordinate care and recognize when adjustments to the plan of care are needed.
• Maintain accurate and timely documentation of all patient interactions, including initial evaluations, treatment progress, and discharge summaries to create a comprehensive record of care.
