Key Responsibilities

• Conduct comprehensive evaluations of patients by reviewing medical histories, performing targeted assessments, and providing clear documentation of findings.
• Engage with patients and their families to explain evaluation results and collaboratively set treatment goals that align with their individual needs.
• Implement evidence-based treatment protocols, adjusting interventions based on ongoing assessments of the patient's condition and progress.
• Collaborate with multidisciplinary healthcare teams to discuss patient status, advocate for necessary modifications in care plans, and ensure optimal outcomes.
• Manage discharge planning and follow-up care; document all aspects of patient care meticulously, including evaluations, treatment plans, and progress notes.
• Utilize effective time management strategies to meet productivity expectations while maintaining an organized patient schedule.
• Contribute to departmental objectives by participating in team discussions, compliance with regulatory standards, and promotional community outreach initiatives.
• Engage in continuous professional development through participation in training and serve as a clinical instructor for internship students, providing mentorship and guidance.

Patient Population / Client Focus

Qualifications

• Bachelor's Degree in Physical Therapy is required; a Doctorate in Physical Therapy (DPT) is preferred.
• A valid physical therapy license in the State of New Jersey is necessary.
• Must hold or acquire American Heart Association's BLS for Healthcare Providers CPR certification.

Preferred Experience

• A minimum of two years of professional experience as a Physical Therapist in a clinical or rehabilitation setting is required.
• Experience with outpatient rehabilitation services is a plus.
