Work Environment

This position is onsite, primarily incorporating home visits to evaluate and rehabilitate patients according to their unique needs. You will be responsible for developing and implementing tailored therapy programs in various home settings, addressing the specific challenges that each patient faces. Key Responsibilities

• Conduct comprehensive evaluations using diagnostic tests to assess patients' functional capabilities, and develop targeted treatment plans to enhance mobility and decrease pain levels.
• Oversee the implementation of therapy strategies executed by Physical Therapy Assistants, ensuring they adhere to established care plans and guidelines.
• Provide hands-on treatment in the patient's home, including manual therapeutic exercises and physical agents such as traction and massage, tailored to individual treatment goals.
• Evaluate and recommend adjustments for prosthetic and orthotic devices to ensure optimal patient performance and comfort.
• Prepare detailed clinical documentation, including initial evaluations, care plans, and discharge summaries, while ensuring compliance with regulatory standards and organizational protocols.
• Engage actively in case conferences with interdisciplinary teams, contributing to comprehensive care strategies that integrate diverse perspectives and expertise.
• Educate patients and their families on physical therapy techniques, caregiving methodologies, and the use of therapeutic devices to promote independence.
• Maintain clear and timely communication with physicians and case managers regarding patient progress and any adjustments to therapeutic strategies.
• Ensure all clinical documentation is submitted promptly, adhering to organizational policies and maintaining comprehensive patient records.

Patient Population / Client Focus

This role entails working with a diverse patient demographic, including older adults, individuals recovering from surgery or injury, and patients with chronic conditions. Your focus on personalized care will greatly affect each patient's recovery process and overall well-being, as you help them navigate the challenges of rehabilitation in their home environment.

Qualifications

• Doctorate in Physical Therapy (DPT) from an accredited institution.
• Current and valid DPT license in the State of New Jersey.
• Certifications in CPR and Basic First Aid.
• Excellent communication skills, with the ability to engage effectively with patients, families, and team members.
• Demonstrated commitment to clinical excellence and patient-centered care.
• Strong organizational skills and attention to detail in documenting patient interactions and treatments.

Preferred Experience

A minimum of two years of clinical experience as a Physical Therapist, preferably in home care or community settings, is highly desirable. Familiarity with OASIS documentation and compliance with federal and state healthcare regulations is an advantage.

Compensation and Benefits

This position offers a competitive salary ranging from USD 100,000 to 120,000 annually, plus comprehensive benefits.
