Home Health Care Physical Therapist (PT) - Full-time

As a Home Care Physical Therapist, you will have the unique opportunity to provide care in a comfortable environment for our clients, which fosters a collaborative approach between you and the families you serve. The hours of operation are Monday to Friday, from 9 AM to 5 PM, allowing for a structured yet rewarding work-life balance.

As a Physical Therapist at Calibre Ortho Care, your responsibilities will include but are not limited to:

Developing comprehensive and personalized treatment plans aimed at preventing disability, relieving pain, and restoring optimal muscle function.

Overseeing physical therapy assistants to ensure that treatment plans are executed accurately and effectively.

Assessing home environments for rehabilitation purposes and recommending modifications to enhance client independence.

Administering prescribed physical therapy treatments including manual therapy, exercise instruction, and functional activities tailored to client needs.

Documenting all clinical interactions, including evaluations, progress notes, and discharge summaries, ensuring compliance with OASIS requirements.

Collaborating with an interdisciplinary team to review and update care plans, contributing to comprehensive client treatment strategies.

Providing education to clients and families regarding therapy progress, equipment use, and care programs.

Maintaining clear communication with physicians and case managers regarding client condition changes and therapy responses.

Completing on-site assessments and participating in community resource evaluations to address client needs appropriately.

Participating in team meetings and case conferences to enhance the overall effectiveness of care provided.

Your work will primarily focus on clients in the homecare environment, ranging from those recovering from surgery or injury to individuals managing chronic conditions. You will be instrumental in guiding each client through their rehabilitation journey, adjusting treatment plans as necessary and ensuring therapeutic goals are met with compassion and dedication.

A Bachelor's Degree from an accredited physical therapy program.

Current and valid PT licensure in good standing within the state of Virginia.

One year of experience as a physical therapist, preferably in acute care or home care settings.

Current CPR certification to ensure safety in emergency situations.

A proactive attitude with strong critical thinking skills to navigate challenging scenarios in patient care.

Previous experience in home healthcare or community-based therapy settings.

Familiarity with electronic medical records and documentation protocols.

Experience with specialized therapies or interventions that could benefit our client demographic.

Calibre Ortho Care offers a competitive salary range of USD 90,000 to 110,000 annually, based on experience and qualifications.
