Work Environment

This full-time position requires you to work onsite in a home care setting, allowing you to engage directly with patients and their families in their own environments. You will be part of a collaborative team of healthcare professionals that values communication and teamwork. Your hours will typically range from 9:00 a.m. to 6:00 p.m., with potential evening clinics until 8:00 p.m. and occasional participation in weekend community outreach events.

Key Responsibilities

• Conduct thorough health assessments and screenings tailored to individual patient needs, ensuring a comprehensive understanding of each patient’s pain management requirements.
• Diagnose and manage both acute and chronic pain conditions, formulating treatment plans that promote preventive care and health education.
• Implement and develop health education programs in partnership with local community organizations, providing valuable resources and information to improve patient outcomes.
• Advocate for patients by addressing barriers to healthcare access and guaranteeing culturally sensitive care, ensuring all patients feel respected and understood.
• Coordinate patient referrals to specialty services and follow up on care plans to ensure seamless continuity of care for all patients.

Qualifications

• Current certification as a Nurse Practitioner.
• An active nursing license in the state of New Jersey.

Compensation and Benefits

We offer a competitive salary range of USD 120,000 to 150,000 annually, commensurate with your experience and qualifications.
